Hospital Medical Records. All records pertaining to the history, diagnosis, treatment or prognosis of a person treated pursuant to the terms of this Agreement which are generated and maintained by the treating hospitals, including subcontractors, except for the discharge summary made a part of the offender’s medical file.

  • Emergency medical care means such medical procedures as:

  • Educational records means: those official records, files, and data directly related to a student and maintained by the school or local education agency, including but not limited to records encompassing all the material kept in the student's cumulative folder such as general identifying data, records of attendance and of academic work completed, records of achievement and results of evaluative tests, health data, disciplinary status, test protocols, and individualized education programs.

  • Emergency medical care provider means an individual who has been trained to provide emergency and nonemergency medical care at the first responder, EMT-basic, EMT-intermediate, EMT-paramedic, paramedic specialist or other certification levels recognized by the department before 1984 and who has been issued a certificate by the department.
