A Net Change in Attachment calculated for the purposes of the Agreement may not be used for the purpose of satisfying any other contractual attachment obligations or commitments which the Clinic may have.
The Clinic Lead(s) will ensure all participating physicians of the Clinic are regularly apprised of the status of the incorporation of the Interprofessional Team Member into the Clinic’s practice, the Net Change in Attachment and the status of the evaluation.
Where the Clinic participates in a PCN and utilizes the services of shared PCN resources as provided under a PCN service plan, the contribution of those resources to the Net Change in Attachment is to be accounted for separately from the contribution of the Interprofessional Team Member.
