Office Visits. Our office policy is to require at least 24 hour’s notice of cancellation of a booked appointment. Failure to provide adequate notice may result in a missed appointment fee from your physician. (Missed new patient appointment: $75. All other visits: $50)

Office Visits. For 2019 a twenty dollar ($20.00) co-pay will be required for each office visit. Co-pays are applied to the calendar year out-of-pocket. Increases to physician services co-pays for 2020and 2021shall not be more than ten dollars ($10.00) per year per visit and the maximum co-pay for physician services shall not increase by more than $25.00 per year. EMERGENCY ROOM SERVICES A seventy-five dollar ($75.00) co-pay shall be required for each emergency room visit. Increases to the co-pay for emergency room visits for 2020 and 2021shall not be more than twenty-five dollars ($25.00) per year. This co-pay shall be waived if the covered plan member is admitted to the hospital.
