Missed Appointment Fee. We require at least 24-hour notice in order to change or cancel any appointments. There will be a $50.00 fee for all appointments missed or canceled with less than 24-hour notice.

Charges for medical records are $1.00 for the 1st page and $0.50 for each additional page. As a courtesy to you, our office will not charge for the first 10 pages, but will begin charging on the 11th page. Th amount of the fee is dependent on the number of pages we need to copy. The charge for copies of photographs or images will be $2.50 per page. You authorize us to include all relevant information, including your payment history. If you are requesting your records to be transferred from another doctor or organization to us, you authorize us to receive all relevant information, including your payment history.
