Missed Appointment Fee. We require at least 24-hour notice in order to change or cancel any appointments. There will be a $50.00 fee for all appointments missed or canceled with less than 24-hour notice.

Missed Appointment Fee. We request that patients give 24 hours notice for canceling an appointment. The charge for copies of photographs or images will be $2.50 per page. You authorize us to include all relevant information, including your payment history. If you are requesting your records to be transferred from another doctor or organization to us, you authorize us to receive all relevant information, including your payment history.

Examples of Missed Appointment Fee in a sentence
