Primary Care. PHYSICIAN (PCP) means, for the purpose of this agreement and for the determination of your copayment, professional providers that are family practitioners, internists, and pediatricians. Nurse practitioners and physician assistants, practicing under the supervision of these professional providers, may be reimbursed as primary care physicians. For the purpose of this agreement, gynecologists and obstetricians may be credentialed as primary care physicians or as specialist physicians.
Primary Care. 2.5.1.1. The ICDS Plan must ensure that each Beneficiary has a Network primary care provider (PCP) of their choice who will serve as an ongoing source of primary and preventive care and will perform care coordination activities appropriate to the Beneficiary’s needs. The PCP’s care coordination responsibilities include, at a minimum, the following:
2.5.1.1.1. Assisting with coordination of the Beneficiary’s overall care, as appropriate;
2.5.1.1.2. Recommending referrals to specialists, non-physician provider types (e.g., nutritionists, physical therapists) and to laboratory, imaging, radiology, and other diagnostic procedures, as necessary;
2.5.1.1.3. Triaging Beneficiaries according to the following:
2.5.1.1.3.1. Emergency care needs must be triaged and treated immediately on presentation at the PCP site;
2.5.1.1.3.2. Persistent symptoms must be treated no later than the end of the following working day after the initial contact with the PCP site; and
2.5.1.1.3.3. Requests for routine care must be seen within six (6) weeks.
2.5.1.1.4. Participating in the development of an ICP, and as a member of the Trans-Disciplinary Care Team.
2.5.1.2. Beneficiaries will be allowed to select a PCP from the ICDS Plan’s network of Providers, and to change the PCP no less often than monthly. Beneficiaries who do not select a PCP will be assigned a PCP by the ICDS Plan prior to the effective date of enrollment.
Primary Care. 2.5.2.1. The PCP must:
2.5.2.1.1. Provide primary medical services, including acute and preventive care;
2.5.2.1.2. Refer the Enrollee, in coordination with the ICT and in accordance with the Contractor’s policies, to Covered Service providers, as medically appropriate; and
2.5.2.1.3. Lead the ICT, together with the Care Coordinator, and if indicated, with the behavioral health clinician
Primary Care. The provision of coordinated, comprehensive medical services on both a first-contact and a continuous basis to a Covered Person. The provision of Primary Care incorporates an initial medical history intake, medical diagnosis and treatment, communication of information about illness prevention, health maintenance, and referral services.
Primary Care. Physician (PCP) means the Participating Physician you select when you enroll, or change to thereafter according to our rules, and who provides or arranges for all your covered health care services.
Primary Care. Members must select a GHC Personal Physician when enrolling under the Agreement. One Personal Physician may be selected for an entire family, or a different Personal Physician may be selected for each family member. If the Personal Physician is not selected at the time of enrollment, GHC will assign a Personal Physician, and a letter of explanation will be sent to the Member. Selecting a Personal Physician or changing from one Personal Physician to another can be accomplished by contacting GHC Customer Service, or accessing the GHC website at xxx.xxx.xxx. Primary Care. Physician is any of Medical Group’s Participating Providers who meet PacifiCare’s criteria for providing initial and primary care Covered Services to Medical Group Members, for maintaining the continuity of patient care, and for initiating and coordinating referrals for Covered Services to Medical Group Members.

Depending on the dispersion of the population served, there will be a range of services provided from practices supporting multiple health, care and wellbeing needs under one roof in or near the local community. Where scale permitted these would typically be referenced as health and wellbeing centres that could accommodate larger primary care practices if required and facilitate interaction between acute and primary care professionals either on a face to face basis or using digital means (e.g. Attend Anywhere) to reduce the need for patients to physically attend higher acute care settings.
