Patient manager definition

Patient manager means an eligible provider who has signed a contract with the department to perform patient management for enrolled recipients.
Patient manager or “PM” is the contracted provider who agrees to accept the terms of this agreement and includes any of the contracted provider’s employees, affiliated providers, agents, or contractors.

  • Outpatient hospital services means preventive, diagnostic, therapeutic, observation, rehabilitation, or palliative services provided to an outpatient by or under the direction of a physician, dentist, or other practitioner by an institution that:

  • Service Manager has the meaning ascribed thereto in Section 2.01(c).

  • Investment Manager or “Manager” shall mean a party furnishing services to the Trust pursuant to any contract described in Section 4.8 hereof;

  • Region Manager means the designated DNR staff person responsible for managing the affairs of DNR in designated large geographic areas. The Region Manager may perform the duties of the Contract Manager.

  • Emergency medical services personnel means that term as defined in section 20904 of the public health code, 1978 PA 368, MCL 333.20904.

  • Contractor Representative means any of the following:

  • City’s Project Manager means the City’s employee, or his/her delegate, who is authorized in writing to deal with the Consultant on behalf of the City in connection with the Services, or to make decisions in connection with this Agreement;

  • Medical provider means a medical service provider, a hospital, a medical clinic, or a vendor of medical services.
