Personally Identifiable Health Information means information that is a subset of health information, including demographic information collected from an individual, and;

More Definitions of Personally Identifiable Health Information

Personally Identifiable Health Information means a natural person’s name used in combination with his or her confidential health care or other medical information, including “protected health information” as defined in the Health Insurance Portability and Accountability Act of 1996, as amended, and any rules or regulations promulgated thereunder.
