Physician Contractor definition

Physician Contractor means a physician who is a Contractor or a Contractor of an Affiliated Company, and who provides radiology or radiology related services to or on behalf of the Company or an Affiliated Company.
Physician Contractor means a physician who is a Contractor of the Company, and who provides radiology related services to or on behalf of the Company.
Physician Contractor means an Eligible Independent Contractor as defined under the Plan or an Eligible PA Employee as defined under the Plan, in each instance who is performing services as a physician.

  • Physician shall establish and maintain procedures and controls so that no medical or enrollee information contained in Physician’s records be used by or disclosed by Physician, Contractor Representatives, or Contractor’s agents, officers, or employees except as provided in Section 1106 of the Social Security Act, as amended, and regulations prescribed there under.

  • If the Participant’s position as an employee or Contractor, including a Physician Contractor, of the Company, of a Subsidiary, or of an Affiliated Company terminates for Cause, all of the Participant’s outstanding Options, whether or not then vested, shall be immediately forfeited back to the Company.

  • Notwithstanding any terms, provision or limitation in any other contract or agreement between a Physician Contractor and the Company or VRC, the right of a Physician Contractor or former Physician Contractor to receive or benefit from an Award under this Plan shall be subject to the limitations and provisions set forth in this Article 13.

  • One or more individuals may be designated as alternate inspectors to replace any inspector who fails to act.

  • Such indemnification by the School Physician Contractor shall apply unless such claims, liabilities, suits, proceedings and actions result from the negligence or misconduct of the School District, its employees, agents or contractors.

Related to Physician Contractor

  • Sub-Contractor means the person named in the Contract for any part of the works or any person to whom any part of the contract has been sublet by the contractor with the consent in writing of the Engineer-in-charge and will include the legal representatives, successors and permitted assigns of such persons.

  • Contractor Personnel means and refers to Contractor’s employees and employees of Contractor’s permitted subcontractors or permitted agents assigned by Contractor to perform Services under this Contract.

A Primary Care Physician’s agreement with HPN may terminate. In the event that a Member’s Primary Care Physician’s agreement terminates, the Member will be required to select another Primary Care Physician.

  • Pharmacist means an individual licensed by this state to compound and dispense drugs, prescriptions, and poisons.

  • Dentist means a registered dental practitioner whose name is included in sub-part A of the first part of the dental list;

  • Physician means a licensed practitioner of medical, surgical, dental, services or the healing arts including accredited Christian Science Practitioner, acting within the scope of his/her license. The treating physician cannot be the Insured, a Traveling Companion, a Family Member, or a Business Partner.

  • Prime Contractor means, in relation to a multiple-employer workplace,

  • Specialist Physician means a licensed physician who qualifies as an attending physician and who examines a patient at the request of the attending physician or authorized nurse practitioner to aid in evaluation of disability, diagnosis, or provide temporary specialized treatment. A specialist physician may provide specialized treatment for the compensable injury or illness and give advice or an opinion regarding the treatment being rendered, or considered, for a patient’s compensable injury.

  • O&M Contractor means the person, if any, with whom the Concessionaire has entered into an O&M Contract for discharging O&M obligations for and on behalf of the Concessionaire;
