Preferred Medication List definition

Preferred Medication List means the listing of preferred medications as determined by AvMed’s Pharmacy and Therapeutics Committee based on clinical efficacy, relative safety and cost in comparison to similar medications within a therapeutic class. This multi-tiered list establishes different levels of Co-payment for medications within therapeutic classes. As new medications become available, they may be considered excluded until they have been reviewed by AvMed’s Pharmacy and Therapeutics Committee.

Peer-reviewed medical literature does not include publications or supplements to publications that are sponsored to a significant extent by a pharmaceutical manufacturing company or health carrier.

  • Medical cannabis means the same as that term is defined in Section 26-61a-102.

  • Prescription medication means any medication that requires a physician prescription before it may be obtained from a pharmacist.

  • Participating Home Infusion Therapy Provider means a Home Infusion Therapy Provider who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • In Patient Care means treatment for which the insured person has to stay in a hospital for more than 24 hours for a covered event.
