Qualified Clinic definition

Qualified Clinic means a fully licensed community clinic or federally qualified health center 16 that has been licensed by the State of California or the Federal Government, has provided services to 17 MSI eligible patients for twelve consecutive months, and has received eligibility identification training 18 by the Hospital Association of Southern California (HASC), Orange County office or by any other 19 means approved, in writing,approved by ADMINSTRATOR. Intermediary from HOSPITAL, Other Providers, or third-party payers.
Qualified Clinic means a fully licensed community clinic or federally qualified health center 5 that has been licensed by the State of California or the Federal Government, has provided services to 6 MSI eligible patients for twelve consecutive months, and has received eligibility identification training 7 approved by ADMINSTRATOR.
Qualified Clinic means any health center, clinic or other non-hospital practice setting in which a physician Employed by Banner Health practices regularly, and is receiving, directly or indirectly, student loan repayment assistance through any governmental program which requires that any physician receiving such student loan repayment assistance must practice in a setting that offers a sliding fee scale for patients based upon the patient’s individual or household income.

  • Non-Participating Certified Clinical Nurse Specialist means a Certified Clinical Nurse Specialist who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Participating Certified Clinical Nurse Specialist means a Certified Clinical Nurse Specialist who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Qualified Census Tract means any Census tract which is designated by the Secretary of Housing and Urban Development as having 50 percent or more of the households at an income level which is less than 60 percent of the Area Gross Median Income in accordance with Section 42(d)(5) of the Code.

  • Rural health clinic means a rural health clinic as defined under section 1861 of part C of title XVIII of the social security act, chapter 531, 49 Stat. 620, 42 U.S.C. 1395x, and certified to participate in medicaid and medicare.

  • Independent educational evaluation means an evaluation conducted by a qualified examiner who is not employed by the public agency responsible for the education of the child in question.

  • Qualifying Educational Program means a program at a post-secondary school level of not less than three consecutive weeks duration that requires each student taking the program to spend not less than 10 hours per week on courses or work in that program.

  • Approved abuse education training program means a training program using a curriculum approved by the abuse education review panel of the department of public health or a training program offered by a hospital, a professional organization for physicians, or the department of human services, the department of education, an area education agency, a school district, the Iowa law enforcement academy, an Iowa college or university, or a similar state agency.

  • Non-Participating Clinical Laboratory means a Clinical Laboratory which does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan provide services to you at the time services are rendered.

  • Qualified city means a city that has incorporated an authority under the municipal lighting authority act, 2012 PA 392, MCL 123.1261 to 123.1295.

  • Drug abuse means any pattern of pathological use of drugs that causes impairment in social or occupational functioning, or that produces physiological dependency evidenced by physical tolerance or by physical symptoms when it is withdrawn.

  • Participating Retail Health Clinic means a Retail Health Clinic which has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Qualified mental health professional means a licensed medical practitioner or any other person meeting the qualifications specified in OAR 309-019-0125.

  • Approved laboratory means a laboratory that is listed in the national conference of interstate milk shipments list of sanitation compliance and enforcement ratings distributed by the United States food and drug administration and as approved by the director.

  • Substance Abuse Professional (SAP) means a qualified person who evaluates employees who have violated a DOT drug and alcohol regulation and makes recommendations concerning education, treatment, follow-up testing, and aftercare.

  • veterinary clinic means a building where animals are given medication or surgical treatment and are cared for during the time of such treatment for financial gain;

  • Licensed clinical social worker means an individual who meets the licensed clinical social worker requirements established in KRS 335.100.
