Treatment of infertility is provided after documentation is received from the physician that includes the plan participant’s reproductive history with test results, information pertaining to conservative attempts to achieve pregnancy, the proposed plan of treatment with CPT codes, and any other documentation as needed.NOTE: This information must be received prior to beginning infertility treatment to ensure maximum benefits.
Elective treatment includes; but is not limited to: • Vasectomy; • Breast reduction; • Sexual reassignment surgery; • Submucous resection and/or other surgical correction for deviated nasal septum, other than necessary treatment of covered acute purulent sinusitis; • Treatment for weight reduction; • Learning disabilities (except Autism Spectrum Disorders); • Temporomandibular joint dysfunction (TMJ); and • Treatment of infertility.
