Acknowledgements and Special Terms. The following acknowledgements and conditions shall be made a part of this agreement: CONFLICT OF INTEREST. To the best of his or her knowledge, Provider certifies that no MSDH employee, or spouse, parent or child of an MSDH employee, serves as a member of its governing body, project staff or has an ownership or pecuniary interest in the Provider. Provider agrees that should this condition change during the period of this contract, Provider shall notify MSDH within 30 days.   • CLOUD SPECIFIC TERMS AND CONDITIONS To the extent that Contractor has received an award for Lot 3, Cloud, the following terms and conditions apply to Lot 3, Cloud.
