Action Plan. Head/School Chair or Xxxx discerns a problem either by direct observation or through information received from other sources, he will meet with the employee to clarify the situation. Action plans will be developed to overcome the identified problem. An action plan will be documented in writing in the departmental files and a copy given to the employee. Such documents will not be considered part of an Employee's Personnel Record. However, the formal appraisal may refer back to the discussion, the action plan and the change, if any, in performance.
