Adjudication Procedures - Identifying ADV's. When a potential abandoned vessel is located, the Code Enforcement Officer (CEO) generates a case and report. The initiating CEO attempts all avenues to identify and make contact with the owner of the vessel. If an owner is located, the CEO works with the owner to get the vessel removed. If the owner is unable to remove the vessel or is identified and not responding to contact attempts, the case is fowarded to the AWAF Coordinator, who ensures that the vessel meets the criteria of 522 H&N. The AWAF Coordinator will then have the AWAF CEO post a notice on the boat requesting anyone having an interest in the vessel to contact the Neighborhood Services Section (NSS). The Coordinator begins the AWAF form. The form requires the following information: Grant Year, Photograph in condition found, photograph of area after the removal of AWAF vessel, document case number, location (waterway name), GPS, description of item to be removed, estimated removal costs, actual removal costs, and inspecting CEO. Once the coordinator determines that the vessel is cleared for removal, the contracted xxxxxx xxxxxx is contacted to have it removed. The City of Stockton and its employees do not conduct any salvage or destruction activities. Vessels are not stored by this agency. The CEO works closely with the contracted marine salver to ensure the correct vessel is removed and that the vessel is completely removed. Local contracts require that all vessels removed must be destroyed or recycled.

  • CLAIM PROCEDURES Claim forms or claim information as to the subject policy can be obtained by contacting Benmark, Inc. (800-544-6079). When the Named Fiduciary has a claim which may be covered under the provisions described in the insurance policy, they should contact the office named above, and they will either complete a claim form and forward it to an authorized representative of the Insurer or advise the named Fiduciary what further requirements are necessary. The Insurer will evaluate and make a decision as to payment. If the claim is payable, a benefit check will be issued in accordance with the terms of this Agreement. In the event that a claim is not eligible under the policy, the Insurer will notify the Named Fiduciary of the denial pursuant to the requirements under the terms of the policy. During this process, T1 traffic may be consolidated onto DS3s and may become unidentifiable to the Carrier. Because resources will be limited, BellSouth may be forced to "package" this traffic entirely differently then normally received by the CLECs. Therefore, a method for identifying the T1 traffic on the DS3s and providing the information to the Carriers is required.
