Allowable Costs and Usage. During the term of this Agreement, COUNTY shall pay CONTRACTOR 27 monthly in arrears, in accordance with the compensation structure outlined in 28 Paragraph 3 of Exhibit B, and Paragraph 2 of Exhibit C, of this Agreement for 1 each referral subject to any exclusions or limitations specified in each 3 this Agreement. CONTRACTOR agrees to supply the services at the unit price 4 referenced above, regardless of the number of referrals from COUNTY.
Allowable Costs and Usage. During the term of this Contract, COUNTY shall pay CONTRACTOR monthly in arrears for each completed Trip as defined in Subparagraphs 2.9 and 2.10 of Attachment A and associated fees as described in Subparagraphs 19.4, 19.5, and

CONTRACTOR agrees to supply the services at the unit price listed stated is Subparagraphs 18.1.1 through 18.1.3 of this Agreement, regardless of the number of referrals from COUNTY.
18.1.1 For the period of July 1, 2016 through June 30, 2017, COUNTY shall pay CONTRACTOR, monthly in arrears, a rate of $52.00 per patch applied with a Pharmchek® Overlay, removed, submitted for analysis, and reported on the laboratory’s website.
18.1.2 For the period of July 1, 2017 through June 30, 2018, COUNTY shall pay CONTRACTOR, monthly in arrears, a rate of $53.00 per patch applied with a Pharmchek® Overlay, removed, submitted for analysis, and reported on the laboratory’s website.
18.1.3 For the period of July 1, 2018 through June 30, 2019, COUNTY shall pay CONTRACTOR, monthly in arrears, a rate of $54.00 per patch applied with a Pharmchek® Overlay, removed, submitted for analysis, and reported on the laboratory’s website.
Allowable Costs and Usage. During the term of this Agreement, COUNTY shall pay CONTRACTOR monthly in arrears, the amount of $1,500 for each SSI, SSP, and/or SSDI application submitted subject to any exclusions or limitations specified in Exhibit A.
21.2.1 For each application, a premium fee will be paid upon approval of benefits. Only one (1) premium fee shall be paid for each approved application in accordance with Subparagraphs 21.2.1.1 and 21.
Allowable Costs and Usage. 22.2.1 During the term of this Agreement, COUNTY shall pay CONTRACTORS participating in TECH services monthly in arrears, at the rate of twenty-five dollars ($25) per day, per bed subject to any exclusions or limitations specified in Exhibit C. No guarantee is given by COUNTY to CONTRACTOR regarding usage of this Agreement. CONTRACTOR agrees to provide TECH services at the unit price listed above, regardless of the number of referrals from COUNTY.

(1) Fee CONTRACTOR bills back to COUNTY after paying a third party urine 4 collection facility, under subcontract, for the collection of a urine specimen 5 from CLIENT, individuals referred by ADMINISTRATOR.
(2) Fee for laboratory analysis of a CLIENT’s urine specimen.
(3) With the exception of Invalid Handling, CONTRACTOR shall obtain written authorization from ADMINISTRATOR prior to providing any service that may incur 9 an Additional Expense to COUNTY.

No guarantee is given by COUNTY to CONTRACTOR regarding usage of this Agreement. CONTRACTOR agrees to supply the services at the unit price listed above, regardless of the number of referrals from COUNTY.
