Amount of contractor fees for ongoing operations activities. (a) The pricing for CONTRACTOR's services described in this Agreement is $12,773,517.05 to be invoiced monthly in accordance with Attachment A to this Agreement. The term of this Agreement is from November 25, 2019 through December 31, 2020. HHSC may extend the term of this Agreement for up to DocuSign Envelope ID: - twenty-four (24) successive one-month periods after the expiration of the initial term, or as necessary to complete the mission of the procurement.

  • REPORT OF CONTRACT USAGE All fields of information shall be accurate and complete. The report is to be submitted electronically via electronic mail utilizing the template provided in Microsoft Excel 2003, or newer (or as otherwise directed by OGS), to the attention of the individual shown on the front page of the Contract Award Notification and shall reference the Group Number, Award Number, Contract Number, Sales Period, and Contractor's (or other authorized agent) Name, and all other fields required. OGS reserves the right to amend the report template without acquiring the approval of the Office of the State Comptroller or the Attorney General.

  • Conflict of Interest Contractor Personnel 2.5.1 The A-E shall exercise reasonable care and diligence to prevent any actions or conditions that could result in a conflict with the best interests of the COUNTY. This obligation shall apply to the A-E; the A-E’s employees, agents, and relatives; sub-tier contractors; and third parties associated with accomplishing work and PROJECTS/SERVICES hereunder.
