Application for Leave and Return. Application for leave of absence will be submitted in writing, will state the specific expiration date of the requested leave, and will be accompanied by a written statement from the bargaining unit member's physician substantiating that an illness or disability exists requiring the requested leave of absence. The application to return shall be made in writing by March 15 and shall be the beginning of the next succeeding school year unless he/she makes a mutual agreement with the Superintendent to return earlier or makes a mutual agreement with the Board to extend the leave for up to one (1) more school year.
