Common use of CMD Form Clause in Contracts

CMD Form. As a condition to this Agreement, Xxxxxx will execute the “Chapter 12B Declaration: Nondiscrimination in Contracts and Benefits” form (Form CMD-12B-101) with supporting documentation and secure the approval of the form by the San Francisco Contract Monitoring Division. Tenant represents that before execution of this Agreement, (i) Tenant executed and submitted to the CMD Form CMD-12B-101 with supporting documentation, and (ii) the CMD approved the form.

Tenant hereby represents that before execution of this AgreementLease, (i) Tenant Xxxxxx executed and submitted to the CMD Form CMD-12B-101 CMD- 12B-101 with supporting documentation, ; and (ii) the CMD approved the such form.
