Conditions and Eligibility for Election of Coverage. a. Subject to the terms and conditions provided under 36.3.b. through 36.3.f., below, an employee shall be entitled to elect the amount of life insurance coverage provided under 36.1., above, upon completion of 180 consecutive (consecutive means without a break of more than five consecutive days) calendar days of active service as a full-time (40-hour per week) employee following his/her initial date of employment with the City. Half-time employees who are employed at least 20 hours per week for 365 consecutive calendar days shall become eligible for Life Insurance on a prorata basis. b. The election of life insurance coverage shall be in a manner prescribed by the City. c. An employee meeting the eligibility requirements for election of life insurance coverage must make such election prior to the date his/her eligibility is first established. If the employee fails to make such election within this time limit, the election shall be made only on such terms and conditions as are established and maintained from time to time by the City and/or its life insurance carrier. d. An employee shall become entitled to the life insurance coverage provided under 36.1., above, the first of the month following his/her eligibility date. e. An employee re-employed subsequent to a separation from active service, for whatever reason, must re-establish his/her eligibility for life insurance coverage on the same basis that would be applicable to a new employee having the same starting date that the re-employed employee had following re-employment. f. Conditions and Eligibility for Election of Coverage. a. The provisions of paragraphs 1 and 2 of this Article, above, shall not apply to Police Services Specialists. The life insurance benefits for Police Services Specialists shall be in accordance with the existing terms of their respective retiree life insurance coverage.
