Contents of the Written Notice. The contents of the written notice shall include, but need not be limited to, the following: 9.3.3.1 A statement, in ordinary and concise language, of the specific acts and omissions upon which the disciplinary action is based; 9.3.3.2 A statement of the cause, or causes, for the action taken; 9.3.3.3 If it is claimed that the unit member has violated a rule or regulation of the District, a statement of the rule or regulation; 9.3.3.4 A statement of the discipline proposed, including beginning and ending date(s), if appropriate; 9.3.3.5 A statement that the unit member may file a request for hearing before the Board of Trustees directly with the Superintendent or his/her designee within five (5) days that the District office is open for business after service of the written notice; 9.3.3.6 A statement that if the unit member does not respond pursuant to 9.3.3.5 above, the District will impose the discipline as noticed.

9.3.3.2 12.5.3.2 A statement of the cause, or causes, for the action taken;.
9.3.3.3 12.5.3.3 If it is claimed that the unit member employee has violated a rule or regulation of the District, a statement of the rule or regulation;.
9.3.3.4 12.5.3.4 A statement of the discipline proposed, including beginning and ending date(sdates(s), if appropriate;.
9.3.3.5 12.5.3.5 A statement that the unit member employee may file file, with the Superintendent or his /her designee, a request for hearing before the Board of Trustees directly with the Superintendent or his/her designee District Governing Board. Such request must be filed within five (5) days that the District office is open for business (see 13.2.3) after service receipt of the written notice by the employee.
12.5.3.6 A form which shall constitute a demand for a hearing signed and filed with the district within (5) working days of notice;.
9.3.3.6 12.5.3.7 A statement that if the unit member employee does not respond pursuant to 9.3.3.5 12.4.3.5 above, the District will impose the discipline as noticed.
