Contract Rates for Skilled Nursing Services Admissions. For the provision of Covered Services to a Customer during an Admission, the Contract Rate is determined as described in this Section 2.2 including Table 1 and Section 2.4 including Table 3, subject to CMS Skilled Nursing Facility (SNF) Consolidated Billing bundling requirements and UnitedHealthcare Policies and Protocols. The Contract Rate for an Admission is the Contract Rate in effect on each day of the Admission. Facility must meet all CMS certification requirements applicable to a Covered Service to be eligible for reimbursement under this payment appendix.
