Covered Services definition

Covered Services means all or a part of those medical and health services set forth in rule 441—86.14(514I).

Covered Services means Medically Necessary health care items and services covered under a Benefit Plan.

More Definitions of Covered Services

Covered Services means the services, supplies, or accommodations for which the Plan provides Benefits.

Covered Services means those health care services which are (i) identified as payable in Certificate(s), (ii) medically necessary as defined in such Certificates, and (iii) ordered and performed by a TRUST PRACTITIONER or other health care PRACTITIONER licensed to order and perform such services.

Covered Services means dental care services for which a reimbursement is available under an enrollee's plan contract, or for which a reimbursement would be available but for the application of contractual limitations such as deductibles, co-payments, coinsurance, waiting periods, annual or lifetime maximums, frequency limitations, alternative benefit payments, or any other limitation.

Covered Services means mental health and substance abuse treatment services reimbursable based on provisions of the Medicaid state plan and paid through the fee-for-service payment system administered by the Iowa Medicaid enterprise.
