Copy of Medical Records. We are happy to provide copies of medical records to you or to a third party that you authorize to receive such copies. We must have a written consent from the patient or patient’s legal guardian for the release of any medical records. The undersigned understands and agrees to pay the charges associated with the cost of time of staff and the cost of the supplies utilized to provide these records. Office charts and records are the legal property of NorCal Endocrinology & Internal Medicine. Prior Authorization Some insurance companies require prior authorization for certain procedures and/or medications. The undersigned understands that NorCal Endocrinology & Internal Medicine is not responsible to have insurance companies approve treatments and/or medications or lower the cost of treatments and/or medications. The prior authorization process can be very time consuming and often requiring lengthy phone calls and completion of multiple forms. We will do our best to try and get prior authorization as a courtesy and the undersigned understands and agrees that a fee may be assessed for a prior authorization request. The undersigned also understands and agrees that we may assist in completing forms such as DMV and disability forms etc. and completion of such forms may also result in additional fees that are not covered by insurance company and are the responsibility of the patient. Notice of Privacy Practices The undersigned acknowledges that he/she has read and understands the Privacy Practices of our office. Initials Prescription refills We, at NorCal Endocrinology & Internal Medicine, refill prescriptions electronically. You may request a refill by contacting our office by phone or through our patient portal. It is imperative that you request refills well in advance of weekends and holidays so you do not run out of medication. Please note that prescription refill requests may take as long as a week to complete.

  • Public Records Taxpayer acknowledges that GO-Biz is subject to the California Public Records Act (PRA) (Gov. Code, § 6250 et seq.). This Agreement and materials submitted by Taxpayer to GO-Biz may be subject to a PRA request. In such an event, GO-Biz will notify Taxpayer, as soon as practicable that a PRA request for Taxpayer’s information has been received, but not less than five (5) business days prior to the release of the requested information to allow Taxpayer to seek an injunction. GO-Biz will work in good faith with Taxpayer to protect the information to the extent an exemption is provided by law, including, but not limited to, notes, drafts, proprietary information, financial information, and trade secret information. GO-Biz will also apply the “balancing test” as provided for under Government Code section 6255, to the extent applicable. Notwithstanding the foregoing, GO-Biz agrees that any information provided to GO-Biz by the FTB, in connection with this Agreement will be treated as confidential tax information protected by Article 2 (commencing with Section 19542) of Chapter 7 of Part 10.2 of the RTC, assuming that FTB can rely on such a section and shall not be disclosed to any party, other than personnel of GO-Biz or the Committee, without Taxpayer’s prior written consent. Taxpayer acknowledges that this Agreement in whole or in part will be made available to the public at least ten (10) calendar days prior to the Committee hearing. Pursuant to RTC sections 17059.2 and 23689, in the event of approval by the Committee of this Agreement, Taxpayer acknowledges and agrees that GO-Biz will post on its website the following information:

  • Inspections, Records, and Cooperation The Owner agrees to provide any information pertinent to this Contract which the Program Administrator, PJ, or HUD may reasonably require. Further, upon reasonable notice to the Owner, Owner agrees to provide access to the Program Administrator, PJ, HUD, or their representatives to the Unit, the property on which the Unit is located, and the Owner’s records (wherever located) relevant to this Contract and compliance with Program requirements.
