Common use of COVID-19 Vaccine Booster Leave Clause in Contracts

COVID-19 Vaccine Booster Leave. Employees who are eligible to receive a COVID-19 vaccine booster may request two (2) hours of COVID-19 Vaccine Booster Leave for the purpose of obtaining an initial COVID-19 booster. Fully vaccinated employees who are at least two months removed from receipt of a Xxxxxxx and Xxxxxxx vaccine or at least six months removed from receipt of the second Moderna or Pfizer vaccine are eligible for COVID-19 Vaccine Booster Leave. To receive COVID-19 Vaccine Booster Leave, employees must provide Human Resources the following: (1) proof of full vaccination; (2) proof of receipt of a COVD-19 booster shot; and (3) a written request on a form prescribed by the Secretary of Budget and Management for COVID-19 Vaccine Booster Leave.
