Request for Leave. At any time, employees may request the use of short-term leave (annual leave, compensatory time use, or personal leave). Such request shall be submitted on the appropriate form and approved or denied on the form within one week of submission to the appropriate authority (practices of shorter time periods will be maintained) except that current practices concerning emergency leave requests shall be maintained. Requests will not be denied unreasonably. The issue of more employees requesting the use of short-term leave than can be granted because of operational needs shall be resolved at the LMC. The Employer shall not request the reasons for the use of earned leave other than sick leave as permitted by Article 12. All leave may be used in tenth of an hour increments provided, however that use of personal leave to cover tardiness related absences shall not serve as a bar or defense to disciplinary action.
Request for Leave. Except as otherwise provided in this Agreement, written requests for leave of absence shall be made by employees prior to the beginning of the period(s) of absence and no payment for any absence shall be made until the leave is properly approved. All leaves of absence without pay shall be granted at the discretion of the Employer and must be approved by the Employer in advance. Upon application by the employee, leaves of absence may be extended or renewed at the discretion of the Employer. The appropriate Leave of Absence Request form is available online at the Human Resources page of the MPS website (xxx.xxxx.x00.xx.xx). A written request must be submitted to the Human Resources Department prior to returning from leave. All returns from leave must be approved by the Human Resources Director or designee.
Request for Leave. Academic employees shall submit an Employee’s Request for Leave/Report of Leave form under Sections B, E and F to their unit administrator.
Request for Leave. If the need for Family Care and Medical Leave is foreseeable, the employee shall provide a written request at least thirty calendar days in advance. If the need for Family Care and Medical Leave is not foreseeable, the employee shall provide written notice as soon as practicable. If the need for Family Care and Medical Leave is due to planned medical treatment or supervision, the employee shall make a reasonable effort to schedule the treatment or supervision to avoid disruption of District operations, subject to the approval of the appropriate health care provider.
Request for Leave. A request for such a leave must be accompanied by a statement from a health care provider certifying the basis for leave (or for additional leave if it is an extension of FML) and that the length of time requested is reasonably necessary given the medical condition.
Request for Leave. Member shall fill out the "Request for Family or Medical Leave Form" prepared by the District to be eligible for leave. This form will enable the District to satisfy its record keeping obligations.
Request for Leave. Request for leaves of absence must be made in writing to the Employer prior to the start of the anticipated leave of absence except where it is impossible to do so. During any leave of absence granted under the provisions of this Section, seniority shall accumulate for the duration of any such leave.
Request for Leave. Upon written request to the employer, an unpaid leave of absence not to exceed one (1) year may be granted by the employer at its sole discretion. The employer's written permission shall specify the dates of departure and return. The Union shall be supplied copies of the request and permission. The employee's seniority, up to the date of departure, shall be maintained but there shall be no accrual of seniority during said leave. An employee taking this leave and returning within the 12-month period shall not be considered to have a break in service.
Request for Leave. Requests for unpaid leaves of absence shall include the reason for the leave along with notification of the beginning and ending dates of the leave.
Request for Leave. Upon written request a leave of absence without pay may be granted to any employee for a period of time not to exceed six (6) months for education, training (which will materially benefit District services), personal or medical reasons where other leave provisions are not available or have been exhausted. An employee shall not be entitled to a leave of absence under this provision as a matter of right, but may be granted only upon the approval of the Fire Chief or designee. A request for a leave of absence in excess of six (6) months may be considered by the Fire Chief, who will evaluate potential benefits to the District, and may grant such extended leave of absence, at the Fire Chief’s sole discretion.
