A data breach can take the form of the following:- • The loss or theft of data; • Equipment failure; • Professional hacking attempt; • Professional “blagging” whereby data is obtained by deceit; and • Human error by accidental disclosure. (An organisation mistakenly providing personal information to the wrong person, for example by sending details out to the wrong address).
