Upon Health Plan’s determination that Provider met or exceeded the performance criteria, Health Plan will request funds from OHA and distribute to Provider within 30 (thirty) days of receiving funds from OHA. The Performance Payment is only dispersed if both Central Oregon and the Columbia Gorge regions have met the criteria as outlined in 8.3.
