The details of the Account that XXXX is authorized to draw upon are indicated on the attached VOID Cheque or banking information that has been stamped by the bank verifying account details. We understand the terms hereof and acknowledge and agree to participate in the PAD Plan with XXXX. We warrant and guarantee that all persons whose signatures are required to sign on Our Account have signed this Authorization below.
