Disclosures Related to Persons Convicted of Crimes. 1. Information regarding the Disclosures Related to Persons (network providers’ owners, persons with controlling interest, agents, and managing employees’) Convicted of Crimes must be provided HHSA:
a. before entering into a contract with the County;
b. at any time upon the County’s or State’s request.
2. The following information must be included:
a. The identity of any person who is a managing employee of the Contractor who has been convicted of a crime related to federal health care programs.
b. The identity of any person who is an agent of the Contractor who has been convicted of a crime related to federal health care programs. For this purpose, the word "agent" has the meaning described in 42 Code of Federal Regulations part 455.101.
3. Information regarding the Disclosures Related to Persons Convicted of Crimes shall be emailed to: XXXXxxxxxxxxx@xxxxxxxxxx.xxx and XXXXXxxxxxxXxxxxxxxxx@xxxxxxxxxx.xxx.

The identity of any person who has an ownership or control interest in the CONTRACTOR or is an agent or managing employee of the CONTRACTOR who has been convicted of a criminal offense related to that person’s involvement in any program under the Medicare, Medicaid, or the Title XXI services program since the inception of those programs. (42 C.F.R. § 455.106.)
17.8.3.3.2. COUNTY shall terminate the enrollment of CONTRACTOR if any person with five percent (5%) or greater direct or indirect ownership interest in the disclosing entity has been convicted of a criminal offense related to the person’s involvement with Medicare, Medicaid, or Title XXI program in the last 10 years.

County shall terminate the enrollment of Contractor if any person with five percent (5%) or greater direct or indirect ownership interest in the disclosing entity has been convicted of a criminal offense related to the person’s involvement with Medicare, Medicaid, or Title XXI program in the last 10 years.
