Doctor Statement Sample Clauses
Doctor Statement. An employee returning to work after an illness or injury may be required to present a statement from his/her physician if the illness or injury requires five or more working days. Abuse of sick leave is cause for dismissal and forfeiture of the lump sum payment.
Doctor Statement. An Employee utilizing a doctor's statement to excuse his absence must furnish it to local management on his first day back to work. Upon receipt of the doctor's statement on the Employee’s first day back, if the Employee has not utilized his allowable number of doctor’s statements under Paragraph C. above, the absence shall be excused. If the contents of the doctor’s statement do not meet the requirements as outlined in Section 1, Paragraph C, Sub-Paragraph 3 of this Article, the Employee will have one (1) working day to correct any discrepancies.

In case of injuries sustained either at work or on the employee's own time, the Board retains the right to require a doctor's verification of the employee's fitness to return to work.
