Eligibility (ADI. The ADI Program addresses the special needs of individuals with Alzheimer’s disease and their caregivers. In order to receive services, the consumer must: (1) Be 18 years of age or older and have a diagnosis of Alzheimer’s disease or related disorder or be suspected of having Alzheimer’s disease or a related disorder. (2) Not be enrolled in a Medicaid capitated long-term care program.
