Delivery of Services to Eligible Clients. The Sub-Recipient shall ensure the provision of a continuum of services that meets the diverse needs of elders andtheir caregivers. Documentation of service delivery must include a report consisting of the following: number of clients served, number of service units provided by service, and rate per service unit with calculations that equal the total invoice amount. The Sub-Recipient shall ensure the performance and reporting of the following types of services, in accordance with the Sub-Recipient’s current AAAPP-approved Applications, the current DOEA Programs and Services Handbook, and Section II.A.

The Sub-Recipient shall not provide direct services to clients, unless specifically authorized in proviso. The services include the following categories: The Contractor shall ensure that core services include a variety of home-delivered services, day care services, and other basic services that are most needed to prevent unnecessary institutionalization. The Contractor shall not directly provide core services to clients. Core services, to be provided at the unit rate identified in the Contractor’s Area Plan, as updated, include the following: Adult Day Care; Chore Services; Companionship; Escort; Financial Risk Reduction; Home Delivered Meals; Homemaker; Housing Improvement; Legal Assistance; Pest Control Services; Respite Services; Shopping Assistance; Telephone Reassurance; and Transportation.

(1) Section 331, Title IIIC1 Congregate Nutrition Services;
(2) Section 336, Title IIIC2 Home Delivered Nutrition Services.
2.1.3.1. Congregate Nutrition Services Nutrition services are provided in congregate settings and are designed to reduce hunger and food insecurity, promote socialization and the health and well being of older individuals by assisting them to gain access to nutrition and other disease prevention and health promotion services. Services include:
(1) Congregate meals;
(2) Nutrition education and nutrition counseling; and
