Eligible Claimants. 4.1. To be an Eligible Claimant under this Agreement, a claimant must be a member of the Resident Subclass or the Non-Resident Subclass of the Class Action.
Eligible Claimants. Any of the remaining unsettled plaintiffs who filed one of five pending lawsuits against Ohio State as of July 29, 2020, pursuant to the Court’s April 17, 2020 Order directing the filing of consolidated complaints, may be eligible to participate in the Program, provided he meets the below criteria and submits information sufficient to allow Xx. Xxxxxxxxx to reasonably confirm all of the following: o That the plaintiff is male; o That the plaintiff attended Ohio State as a student during the period Xxxxxxx was employed by Ohio State (i.e., September 1, 1978 to February 28, 1998); o If a student-athlete, the sport(s) in which the plaintiff participated; o That the plaintiff was examined by Xxxxxxx during the time period of the plaintiff’s student attendance at Ohio State; o That the plaintiff was abused or harassed by Xxxxxxx during the time period of the plaintiff’s student attendance at Ohio State; o That the information the plaintiff provided in support of his claim is true; o That the plaintiff did not participate in either of the settlements of March 6, 2020, or October 12, 2020; and o That the plaintiff filed his claim against Ohio State as of July 29, 2020, pursuant to the Court’s April 17, 2020 Order directing the filing of consolidated complaints in one of the following five pending cases:

  • Contractor Hearing Board 1. If there is evidence that the Contractor may be subject to debarment, the Department will notify the Contractor in writing of the evidence which is the basis for the proposed debarment and will advise the Contractor of the scheduled date for a debarment hearing before the Contractor Hearing Board.

  • Hearing Aids Any active employee who is insured under any one of the 9 District sponsored medical plans may request reimbursement for the costs of 10 hearing aids. The maximum amount of reimbursement shall not exceed one 11 thousand dollars ($1,000) within any three (3) year period. The cost of 12 hardware, fitting tests, and other tests related to the hearing aids purchased 13 shall be included for reimbursement purposes. 14

  • Submitting False Claims The full text of San Francisco Administrative Code Chapter 21, Section 21.35, including the enforcement and penalty provisions, is incorporated into this Agreement. Pursuant to San Francisco Administrative Code §21.35, any contractor or subcontractor who submits a false claim shall be liable to the City for the statutory penalties set forth in that section. A contractor or subcontractor will be deemed to have submitted a false claim to the City if the contractor or subcontractor: (a) knowingly presents or causes to be presented to an officer or employee of the City a false claim or request for payment or approval;

  • Settlement Class Members “Settlement Class Members” shall mean all persons in the Class who do not exclude themselves pursuant to Section F, herein, and those who submit a Valid Claim.
