Enrollment Period. Educational Support Professionals may elect to participate in the Career Transition Trust annually during a two (2) week enrollment period determined by the District, but that will occur no later than May 1st each year, provided they have met the eligibility requirements for participation in Subdivision. 2.
Enrollment Period. Employees may start 403(b) or 403(b)(7) plan at any time, however, the deductions will begin after paper work has been submitted to the TPA and according to TIME FRAME FOR DEDUCTIONS.
Enrollment Period. The period of time when a tow company may submit an application for inclusion on a sheriff’s station’s rotation tow list. The enrollment period will be open from March 1 through March 31 of each year.
Enrollment Period. Each Member shall be enrolled with a Contractor subject to meeting applicable Medicaid eligibility requirements. Enrollment with the Contractor begins at 12:01 a.m. on the first calendar day of the first calendar month for which the Member's name appears on the Member Listing Report, and is automatically renewed for twelve (12) months unless the Member becomes ineligible for the program and is disenrolled. The Division shall provide Members with open enrollment periods in accordance with program enrollment requirements and the length of Member enrollment. The first ninety (90) days following a Member’s initial Enrollment will be an open Enrollment period during which the Member can enroll once with a different Contractor without cause. Members subject to either mandatory or optional Enrollment into MississippiCAN are subject to the enrollment requirements as outlined below:
1. Enrollment of Populations with the Option to Disenroll: The Division will enroll newly eligible Medicaid beneficiaries into MississippiCAN. Beneficiaries who are in eligibility categories that may voluntarily participate in MississippiCAN will have the option to disenroll from the MississippiCAN Program without cause or change Contractors without cause during the ninety (90) day period following the date the Division sends the Member notice of Enrollment or the date of the Member’s initial Enrollment, whichever is later; during the annual open Enrollment period; upon automatic reenrollment if the temporary loss of Medicaid eligibility has caused the Member to miss the annual Disenrollment opportunity; or when the Division imposes an intermediate sanction on the Contractor as specified in this Contract in Section 16, Default and Termination, of this Contract.
Enrollment Period. 4.2.10.1 After enrolling in the CONTRACTOR’s MCO (whether as the result of selection or assignment, Members shall have one (1) opportunity anytime during the three (3) month period immediately following the effective date of enrollment with the CONTRACTOR’s MCO to request to change MCOs. After exercising this right to change MCOs, a Member shall remain enrolled with the MCO until the annual choice period described in Section 4.2.10.2 of this Agreement, unless the Member’s change of enrollment is permitted in accordance with NMAC 8.308.7.9.H or the Member is disenrolled in accordance with Section 4.3 of this Agreement and NMAC 8.308.7.10.
Enrollment Period. 4.2.9.1 After enrolling in the CONTRACTOR’s MCO (whether as the result of selection, assignment, or auto assignment), Members shall have one (1) opportunity anytime during the three (3) month period immediately following the effective date of enrollment with the CONTRACTOR’s MCO to request to change MCOs. After exercising this right to change MCOs, a Member shall remain enrolled with the MCO until the annual choice period described in Section 4.2.9.2 of this Agreement, unless disenrolled in accordance with Section
Enrollment Period. The Enrollment Period for your Loyalty Program tier will last for 12 months or until you accept an invitation to enroll in a higher tier, whichever is sooner. The Enrollment Period will begin on the following day after the day in which you pay the applicable Annual Fee or, if you were previously enrolled in the same tier, the following day after the day in which you renew your enrollment in that tier.
Enrollment Period. The enrollment period shall be determined by KCCD in accordance with its guidelines, policies, pertinent statutes, and regulations.
Enrollment Period. The above option elections must be made by the teacher in writing annually during the enrollment period, or in the case of newly hired teachers within thirty (30) calendar days of the beginning of employment. Insurance coverage for such employees who enroll and otherwise qualify will become effective as the terms of the insurance policies and the regulations of the insurance carrier permit. Teachers who fail to complete the necessary paperwork during the designated enrollment period may have their coverage discontinued and forfeit their coverage, provided the District communicates all deadlines to the individuals and to the Association in writing. The open enrollment process may be done electronically at the option of the district.
Enrollment Period. 1. An “annual” enrollment period shall be held at a time mutually agreed upon by the District and the Association. During the enrollment period, any employee previously eligible for benefits who had not enrolled in one of the Board provided health- care options will be permitted to enroll in such a plan, subject to carrier provisions. During the enrollment period, dependents previously eligible for benefits who had not enrolled in one of the Board provided health- care options will be permitted to enroll in such a plan.
2. No changes in the insurance selection will be made by the employee during the year except due to an I.R.S. qualifying event.
3.a. Upon return from a Board approved leave without pay the employee shall be entitled to re-enroll in the benefit plans they were previously enrolled in prior to the leave of absence. The employee has 30 days from their return from leave to submit the enrollment forms to the Employee Benefits Department. Coverage will be effective the first of the month following 30 days after the return from leave.
