EXISTING CONDITION LIMITATION. If someone sought medical attention or had symptoms of an illness three (3) months prior to enrolling in the coverage, the disability will not be covered for the first twelve (12) months of coverage. APPENDIX F—DISTRICT PROVIDED LONG TERM DISABILITY COVERAGE ELIGIBILITY—All SEALS members will have ability to access this benefit. MINIMUM HOURS—20 hours. PREMIUM CONTRIBUTION—This benefit will be 100% employer paid. PERCENT OF SALARY—The benefit will cover 60% of salary. MONTHLY MAXIMUM—$5,000.00. MINIMUM BENEFIT—$100.00 per month. ELIMINATION PERIOD—90 calendar days (13 weeks). INTEGRATION WITH SICK LEAVE—Up to one (1) year of available sick leave may be used in combination with long term disability benefit in order to receive 100% of salary during this time period. INTEGRATION—Full family.
