Extension of Eligibility. Alliant and the Employer may together agree to the extension of eligibility to persons other than those provided for in the Certificate of Coverage. Any such extension of eligibility shall be in writing on the Employer Enrollment Application.

  • Determination of Eligibility The Plan Administrator shall determine the eligibility of each Employee for participation in the Plan based upon information provided by the Employer.
