Final Notice of Disciplinary Action. a. The Final Notice of Disciplinary Action shall include the following: 1) The disciplinary action taken by the Board’s designee; 2) The effective date of the disciplinary action; 3) Specific charges upon which the action is based; 4) A factual summary of the facts upon which the charges are based; 5) The unit member’s right to appeal.
