Common use of Group Summary Clause in Contracts

Group Summary. A Group Summary, which is included in the Attachments and periodically sent to Practitioner, and incorporated herein by reference, is submitted by HWHN to Practitioner. The Group Summary provides for the rendering of Complementary/Alternative Health Services to Participants with respect to a specific Group. Each Group Summary shall identify pertinent terms, conditions, and requirements and payment arrangements for the rendering of Services under such Group Plan.
