Common use of Health and Dental Clause in Contracts

Health and Dental. (a) A group hospital, surgical, major medical and dental plan as agreed to by the parties shall be available to employees. In the event the Employer shall propose a change in this plan, this Contract shall be reopened for purposes of negotiations on such a proposed change. For group health insurance for 2010, the Employer shall pay up to four hundred eighty four dollars and eighty cents ($484.80) per month for employees desiring the "single plan" and up to one thousand one hundred thirty-nine dollars and twenty-nine cents ($1139.29) per month for employees desiring the "family plan". Employees with a spouse on Medicare Plus will receive a payment not to exceed that paid by the Employer for family coverage. If the health insurance premiums are raised during the term of this agreement, the dollar contribution caps cited herein shall be adjusted by the percentage equal to the increase in the premiums for the point of enrollment plans. The health insurance plan shall authorize coverage for the domestic partner of an employee. Domestic partner shall mean partner of the County employee who is a person of the same or of the opposite sex, which relationship meets the criteria and is set forth in this Agreement. The Employer agrees that employees and their dependents selecting a health care provider offering Dane County both the Point of Enrollment/Point of Service and HMO plans will be allowed one (1) thirty (30) day enrollment period per year during which time an employee enrolled in the plan specified above can choose between that provider’s XXX/POS or HMO. For group dental insurance for 2010 the Employer shall pay up to forty dollars and seventy- nine cents ($40.79) for employees desiring the "single plan", up to one hundred fourteen dollars and ninety-nine cents ($114.99) per month for those desiring the "family plan". The dental insurance plan shall authorize coverage for the domestic partner of its employees. Such coverage shall be on the same basis as those participating in the family plan. Domestic partner shall mean partner of the County employee who is a person of the same or of the opposite sex, which relationship meets the criteria and is set forth in this Agreement. For the first four (4) years of such retirement, the Employer will pay a maximum of ninety dollars ($90.00) per month toward the premium cost of the retired employee, surviving spouse and any eligible dependents, with the Employer applying any accrued sick leave credits of the retired employee as provided in Article XV, Section 6 (a), to any excess cost over the ninety dollars, ($90.00) per month. Retired employees, without sick leave credits, may pay this difference or full cost of premium or subscription after four (4) years of retirement and continue in either plan to age sixty-five (65) by making payments of the difference or full cost to the Employee prior to the tenth day of the month preceding the month of coverage. [The provisions of this paragraph may be delayed for the same period as detailed in Section 15.06(a)].
