Common use of Health Care Provider’s Letter Clause in Contracts

Health Care Provider’s Letter. If requested by the Assistant Superintendent for Human Resources, the employee shall furnish a letter from a health care provider. Generally, this note is not requested until after the fourth consecutive day of absence. This letter should state the nature of the illness and the anticipated period of absence.
