Home Health Care Agency. a Facility Provider or Hospital program for home health care, licensed by the state and certified by Medicare which, for compensation from its patients: a. provides skilled nursing and other services on a visiting basis in the Member’s home, and b. is responsible for supervising the delivery of such services under a plan prescribed by the attending Physician.
