Catering The IU Auditorium staff may suggest caterers for your event; however, the Licensee will be responsible for paying all catering costs in a timely manner. All caterers in the IU Auditorium must be pre-approved by IU Auditorium Management and possess all valid Indiana licenses for food service and the service of alcoholic beverages (if applicable). The caterer must carry commercial general and auto liability insurance, including a products-completed operations endorsement, with minimum limits of $1,000,000 per occurrence / $2,000,000 Aggregate. If alcohol is being served the caterer must also carry dram shop liability with minimum limits of $3,000,000. Licensee must be in compliance with all other university regulations regarding food service including, but not limited to, completing the University Office of Environmental Health & Safety’s Temporary Food Service application, if necessary. Also, “The Trustees of Indiana University, its officers, agents and employees” must be named as an additional insured on the Certificate of Insurance for both Commercial General Liability and Automobile Liability coverage. The Certificate of Insurance must be submitted to Indiana University for review and approval at least fourteen (14) working days prior to the scheduled date of the event. If Licensee’s vendor/caterer fails to comply with any of the requirements described in this section, the vendor/caterer will not be permitted to serve food or alcohol at the Event and Licensor shall not be responsible for any losses incurred by Licensee or Licensee’s subcontractors as a result of such cancellation. Stage Labor IU Auditorium has an agreement with IATSE Local 618 to provide audio services for all functions in IU Auditorium facilities. Sound and/or lighting requirements must be communicated in writing to the Auditorium Management a minimum of one week before the scheduled event. Sound System If the Licensee has contracted for audio services through another agent (band, outside company, etc.), IU Auditorium reserves the right to assign a member of IATSE Local 618 to be present for the event, in accordance with the governing labor agreement. Box Office Charges IU Auditorium reserves the right to provide ticketing services for all public and private events held at IU Auditorium. All customary charges, including credit card commissions, ticket printing, outlet, and phone commissions are included in the daily facility fee. The final decision as to when tickets will go on sale will be made by the Auditorium Director of Ticketing and Sales in cooperation with Ticketmaster and the Licensee. Facility Fee IU Auditorium collects a facility fee from each ticket sale for any event held at IU Auditorium and places it into a special reserve account. Funds in this account are used for facility capital purchases and improvements. The funds are also used to cover the costs of extraordinary repairs and maintenance. This fee is $3.50 for events where the top ticket price is $15.01 or higher. The fee is $2.50 for events where the top ticket price is $15.00 or lower. Pre-box Ticket Sales IU departments or organizations must first complete a Revenue Producing Activity training before pre-box tickets may be sold by any members of the organization. Non-IU departments or organizations may only sell pre-box tickets using cash or check.
Playground ~ The playground is primarily for the use of children under the age of twelve (12). Any child caught throwing rocks, vandalizing, or hitting or teasing other children will be asked to leave the playground. An adult or appropriate baby-sitter must accompany all children under the age of six (6) years old. Garbage ~ The storage, collection and disposal of refuse and garbage in the recreational camping areas shall be conducted as to create no health hazards, rodent harborage, insect breeding areas, accident or fire hazards, or air pollution. Bag all food garbage in plastic and tie shut before placing in the dumpster, to prevent odor and rodents which are drawn to the garbage area, such as skunks, raccoons etc. Dumpster is for normal daily household garbage, no large boxes, building supplies, carpets etc. Camper is to dispose of this type of garbage elsewhere. Garbage may be placed in dumpsters only. If a dumpster is full, do not place garbage on or around the dumpster, find an empty dumpster to place your garbage into. Bags of garbage may not be set outside your camper for any reason. Disposal or deposit of hazardous or toxic waste is prohibited. No gas, oil, batteries, or car fluids may be deposited into the dumpsters or around the dumpsters. Hazardous materials will not be disposed of by the garbage service. Disposal or deposit of furniture, barbeque grills, washer, dryers, and other large household items are NOT permitted on, in or around the dumpsters. Holding Tanks ~ There is no size restriction on holding tanks, however, they must be opaque XX xxxx/black water approved tanks. The on-site dump station may be used to empty tanks that hold 50 gallons or less. For tanks over 50 gallons Camper must contract with a pumping service to pump and remove tank contents from campground. No dumping of xxxx water onto the ground. Docks ~ Campground does NOT have a lifeguard on duty. The docks are privately owned. In order to be on someone's dock you must receive permission from the dock owner. An adult must accompany all children on the docks after dark. Life jackets are required for young children fishing off docks. Running, diving, and/or horseplay are not allowed on the docks. All debris and toys must be removed from the docks, beach and marina area. Inappropriate behavior, intoxication or shouting on docks is not allowed. Glass is not allowed on the beach area. Jet skies, wave runners, etc. are not permitted in the marina area. Tires cannot be left in the marina area under boats or tied onto docks. Fish House ~ Clean up fish cleaning facility when done with your fish cleaning and bag all fish remains. Fish cleaning house must be kept clean and all fish waste must be put into plastic bags and deposited into dumpster and cover on dumpster closed. No dumping of fish wastes into marina or dog waste stations! Swimming Pool ~ Campers are required to abide by all rules posted in and around the pool area.

Footwear Where an employee is required by the College or by legislation, in order to perform his/her duties, to acquire and wear protective footwear, the employee shall provide the College with proof of purchase by March 1 each year and the College shall reimburse such employee, on the first pay day in April in each year, up to a maximum of one hundred and fifty dollar ($150.00). In situations other than the foregoing, the College may, in its discretion, (which discretion shall not be unreasonably exercised) reimburse such expense where it is recommended by the health and safety committee constituted under the Occupational Health and Safety Act.
Access Management The Engineer shall coordinate and evaluate access management within the project limits in accordance with the latest State Access Management Manual or as directed by the State.
Ambulance Services Ground Ambulance This plan covers local professional or municipal ground ambulance services when it is medically necessary to use these services, rather than any other form of transportation as required under R.I. General Law § 27-20-55. Examples include but are not limited to the following: • from a hospital to a home, a skilled nursing facility, or a rehabilitation facility after being discharged as an inpatient; • to the closest available hospital emergency room in an emergency situation; or • from a physician’s office to an emergency room. Our allowance for ground ambulance includes the services rendered by an emergency medical technician or paramedic, as well as any drugs, supplies and cardiac monitoring provided. Air and Water Ambulance This plan covers air and water ambulance services when: • the time needed to move a patient by land, or the instability of transportation by land, may threaten a patient’s condition or survival; or • if the proper equipment needed to treat the patient is not available from a ground ambulance. The patient must be transported to the nearest facility where the required services can be performed and the type of physician needed to treat the patient’s condition is available. Our allowance for the air or water ambulance includes the services rendered by an emergency medical technician or paramedic, as well as any drugs, supplies and cardiac monitoring provided.
