Incidents. The Provider must notify the Department as soon as possible, and at the latest within 24 hours, of any incident relating to any Activity, including: any accident, injury or death occurring during, or as a result of, the specified activities, including in relation to a Participant or a member of the public; any incident which relates to a work, health and safety issue; and any incident that may negatively impact upon the Disability Employment Services or bring the activities into disrepute.
