Information on Plan Enrollment. (a) The Participating Employer shall distribute, either paper distribution or electronically (ex. Employee portal), benefit communication material as provided by the Third Party Administrator to existing Participating Employees within 10 days of receipt of such material; and (b) The Participating Employer shall distribute, either paper distribution or electronically (ex. Employee portal), benefit communication material as provided by the T ▇ ▇ ▇ ▇ P a r t y Administrator to all new Participating Employees within 10 days from the date of hire. (c) The Participating Employer shall complete and submit the Trust’s Waiver of Life Insurance Premium Plan Administrator Statement to the Trust for life waiver claims when the Trustees do not administer and adjudicate the LTD benefits. (d) Each Board shall maintain existing beneficiary declarations. When required, the Board shall provide the most recent beneficiary declaration on file to the Administrative Agent.
