Common use of Insurance Application Clause in Contracts

Insurance Application. Subd. 1: An employee on unpaid leave under the provisions of this Article is eligible to continue to participate in group insurance programs if permitted under the insurance policy provisions but, unless otherwise provided in this Master Agreement, shall pay the entire premium for such programs as he/she wishes to retain, commencing with the beginning of the leave. It is the responsibility of the employee to make arrangements with the Human Resources Office to pay to the District the monthly premium amounts in advance and on such date as determined by the District. The right to continue participation in such group insurance programs, however, will discontinue upon termination of employment except as required by law.
