INSURANCE PLAN RESTRICTIONS Sample Clauses
The Insurance Plan Restrictions clause defines the limitations and conditions that apply to the use or coverage of an insurance plan. It typically outlines what types of services, treatments, or providers are excluded or subject to special requirements, such as pre-authorization or network limitations. For example, certain elective procedures may not be covered, or coverage may only apply if care is received from approved providers. This clause serves to clarify the boundaries of insurance coverage, helping both the insurer and insured understand what is and is not included, thereby reducing disputes and unexpected costs.
