Leave Procedures. Leave requests shall be submitted to the appropriate supervisor on a “Request for Excused Absences for Hourly Employees” form.
Leave Procedures. When a bargaining unit member is absent for any reason, that bargaining unit member must submit the leave through the District’s on-line system. The bargaining unit member must also provide appropriate documentation for the leave, other than sick and personal leave, before it will be approved.
Leave Procedures. Nurses may request leaves of absence. Each request shall 4 be judged by the Department Director on its individual merit and on the basis of the guidelines 5 provided herein. 6 7 No leave of absence, with or without pay, shall be granted unless a request is submitted in 8 writing by the nurse to the Department Director. With the exception of emergencies 9 relating to Family Medical Leave, approval of leaves of absence shall be obtained prior to 10 beginning the leave. All paid leaves shall be considered time worked. Leaves of absence 11 without pay shall not be considered time worked, and the nurse shall not accrue benefits, 12 seniority, or other conditions noted herein during leaves without pay.
Leave Procedures. Unpaid leaves are subject to the following:
A. Leave requests shall be submitted to the Director of Transportation on a “Request for Excused Absences for Hourly Employees” form.
B. Requests for unpaid leaves must be made in writing to the Assistant Superintendent for Human Resources at least ten (10) working days prior to the anticipated date of the leave unless unanticipated circumstances make 10-day advanced notice impossible. Requests shall include the reason for the leave and the beginning and ending dates of the leave. The Director of Transportation or Assistant Superintendent for Human Resources shall respond to the leave request within ten (10) working days of receipt.
C. All leaves in this article must be approved in advance by the Assistant Superintendent for Human Resources.
D. At least thirty (30) working days prior to the date a leave is scheduled to expire, an employee shall notify the Director of Transportation of his/her intent to return to work. Failure to do so shall constitute a “voluntary quit,” and shall terminate all employment rights, provided that prior to the termination of any rights, the Director of Transportation has contacted the employee to determine the employee’s intent to return.
E. Except for short term leave (Section 20.05), leave shall only be granted to employees with one (1) or more years of seniority.
F. Employees who are granted and use unpaid leave on the day(s) preceding or following a paid holiday will not receive holiday pay for that holiday.
Leave Procedures. A. An A&S staff member whose certificate status is current will be considered as if actively employed by the Board during paid leave and will be placed on the salary schedule at the level the employee would have achieved if the A&S staff member had not been absent.
B. All benefits to which an A&S staff member was entitled at the time the leave of absence commenced, including unused accumulated sick leave and credits toward sabbatical eligibility, will be restored to the A&S staff member upon return. The A&S staff member will be assigned to a substantially equivalent position, if available, and for which the A&S staff member holds a valid certificate.
C. The A&S staff member shall have the option of continuing as a participant in the existing insurance benefit programs at the expense of the A&S staff member while taking leave under any provision of this Article.
D. All requests for extended leaves of absence, extensions, or renewals of such leaves, will be in writing and the Board will make a written response to all such requests. A&S staff members on extended leave must give notice of their intention to return to work by April 1.
E. Unless mutually agreed upon, employees taking leave under any provisions of this Article will return commencing July 1 of that year.
Leave Procedures. An employee shall, other than for an emergency leave, apply for a leave of absence in writing and through the immediate supervisor as soon as possible, but not less than by thirty (30) workdays in advance of its requested starting date. A leave of absence application will be processed promptly so that the Board's written decision on it may be rendered by at least five (5) workdays prior to the requested leave starting date. If circumstances preclude the Board from rendering a decision on the leave by this time, the employee will be notified and kept informed of the status of the leave application. If requested, the reason(s) for denying a leave will be given to the employee in writing.
Leave Procedures a. Employees requesting a leave must give the Human Resources Department no less than fourteen (14) calendar days’ notice prior to their projected leave start date, except in emergencies. At time of requesting a leave the Employee must also indicate a projected return to work date.
b. The position held by an Employee who is granted a leave of absence will be filled in accordance to the temporary assignment language in Article 13-Vacancies and Temporary Assignments-Section C. Employees filling the temporary assignment must meet the qualifications of the positions as outlined in the job description of the assignment.
c. If the Employee returns to work within six months year from the last day worked, they will return to the position held on their last day worked. if the employee's leave extends beyond six months or his/her position is filled permanently from their last day worked, their original position is not guaranteed, and they could be returned to a vacant position in the job classification they previously held for which they are qualified. Refusal to accept such assignment will be considered as a voluntary resignation.
d. If the Employee’s return to work date changes from the Employee’s original request for leave they must notify the Human Resources department of the change. At least seven (7) calendar days’ notice must be given prior to the initial projected return to work date for non-medical leave requests.
e. Failure to provide notice of intent to return to work as provided herein, and/or to return to work as scheduled, will release the School District of any further responsibility for re- employment of the employee and shall act as a conclusive presumption that the employee has resigned.
f. Employees on an unpaid leave not covered by FMLA will have all wages and benefits discontinued for the duration of the unpaid leave except the employee may make arrangements to pay the premium of life and health insurance at the prevailing group rate if allowable.

Leave Procedures. 10.4.1 Teachers shall be credited with the full annual leave allowance at the beginning of the contract year.
10.4.2 Annual leave for less than full-time teachers will be prorated.
10.4.3 In the event that the contract is terminated prior to the termination date stated thereon, and the teacher has used more days leave than he/she has accumulated (at the rate of number of days worked divided by the number of days contracted times 12 days), a per diem deduction therefor will be made on the teacher’s final paycheck.
10.4.4 A teacher taking annual leave is not required to state the reason for taking such leave and his/her signature on the District’s leave form or entry of absence into the online absence reporting system will be the teacher’s certification that the leave is being taken under the conditions of this Article; provided, however, that except for a request for annual leave for illness, a request for annual leave for a non-student contact day, a day preceding or following a school holiday or vacation period, or the day preceding or following a non-student contact day if the teacher will not be working a full day on the non-student contact day itself, must be approved in writing by the teacher’s principal and the Director of Human Resources at least 24 hours prior to the time requested for leave. However, in an emergency, requests for approval may be made orally by the principal or the Director of Human Resources. The teacher should provide the teacher’s principal notice of the need for such leave as soon as the need becomes known.
10.4.5 The teacher shall notify the Director of Human Resources in writing of any anticipated use of annual leave for elective surgery, pregnancy or other types of medical conditions which are predictable in advance as soon as the teacher becomes aware of the need for such leave but, if possible, at least 45 calendar days in advance of the anticipated commencement of such leave. The teacher shall also include a physician’s statement with such notice certifying such medical condition. The teacher shall give the Director of Human Resources at least 10 working days’ advance notice of the date on which the teacher intends to return to work.
10.4.6 Except in case of illness pursuant to 10.2 above, a request for annual leave for four or more consecutive school days must be approved in writing by the Director of Human Resources. Leave requests may be submitted no more than 30 days in advance; however, an exception for extenuating c...
Leave Procedures. 1. Where an Administrator is absent for a reason other than those stated in sections above, and sections (a) through (c) of ARTICLE 7, or, except in the case of illness, is absent for a period in excess of the limits established in those sections, there shall be deducted for each day of absence not provided for in those sections and for each day of absence in excess of the prescribed limits, an amount equal to the Administrator’s daily rate of pay.
2. The Superintendent may, in exceptional circumstances, grant a leave of absence with pay for other reasons or may extend the time limits for a period greater than the time limits as set forth in this Article.
