Common use of Length of Medical Leave Clause in Contracts

Length of Medical Leave. A medical leave of absence will be granted for a period not to exceed six (6) months duration from the last day of work. All such leaves of absence require specific approval from the Human Resources Director. a. An employee is not permitted to take more than a total of six (6) months of combined medical, FMLA and family leave. Family leave may not exceed ninety (90) days beyond accrued paid vacation . b. An employee may request an extension of a medical leave of absence from the Human Resources Director. The request must be made in writing and submitted with a statement from a physician acceptable to the Library at least seven (7) days prior to the expiration of the original leave of absence request.
