Long Term Disability insurance will be provided on a flat monthly benefit of eight hundred ($800.00) dollars, payable upon certification of disability and following a thirty-two (32) week waiting period, all subject to the terms and conditions set out in the Sun Life Insurance Group Policy #56276.
