Common use of Major Restorative Clause in Contracts

Major Restorative. Notwithstanding orthodontic maximums, eligible participants shall be entitled to a maximum of two thousand five hundred dollars ($2,500) reimbursement per calendar year for eligible dental claims. This maximum includes major restorative (crowns, caps, bridges) based upon a 50/50 co- payment. Effective August 21, 2015, the maximum two thousand, five hundred dollar ($2,500) reimbursement per calendar year shall include dental implants based upon a 50:50 co-payment. Effective January 1, 2021, notwithstanding orthodontic maximums, eligible participants shall be entitled to a maximum of three thousand dollars ($3,000) reimbursement per calendar year for eligible dental claims.
